Informed Decision Making In CLTI
This session will be helpful in expanding physician knowledge base and improving clinical confidence and effectiveness in treating patients with CLTI. This session will focus on shared, patient-centered decision making to individualize treatment recommendations. The importance of staging and setting patients’ expectations to facilitate decision making for CLTI patients will be reviewed.
This recording also includes the live patient panel held at VAM24
Funding for this activity was provided by a grant from CMSS aimed to promote diagnostic excellence across the field of medicine.
Learning Objectives
- Review the PLAN (Patient risk, Limb severity, ANatomic complexity of disease) algorithm for shared, patient-centered decision making in chronic limb threatening ischemia (CLTI).
- Learn how to manage patient expectations throughout the limb preservation process.
- Learn how to present appropriate options to your patients and customize information to meet patient goals and avoid undesirable outcomes.
- Be able to describe approaches to shared decision making in complex CLTI scenarios, including consideration of palliative wound care.

    Scheduled for this surgery on 5/29/2024. No family history of aneurysm. However I did smoke for 34 years. I'm 53 with history of early deaths on maternal and paternal sides of family. I'll be 54 on 21st of this month.

    This happened on Thursday 10th November 2010. At the time 🕰️ , I was aged 34 years old that I used to smoke 🚬 💨 tailor made cigarettes 🚬 which it’s absolutely 👍 💯 disgusting 🤢 🤮 . Yuck 🤢 🤮 . I’m not returning back to smoking 🚬 cigarettes 🚬 again because I’m worried 😦 that it can stop ✋ the blood 🩸 going into my legs 🦵 🦵 , makes me cough 😷, makes my clothes smell 👃 disgusting 🤢 🤮 , causes heart ❤️ failure 😞 😣 😨 , lungs 🫁 go black ⬛️ , damage my teeth 🦷, put me in hospital 🏥 and finally could pay 💰 for treatment and perhaps 🤔 it would serve me right. Be healthy to myself is please 🙏 don’t smoke 🚭 cigarettes 🚬 again anymore is that there is an issue warning ⚠️ ‼️ ⛔️ and I’m not joking 🙃. Stay well away from smoking 🚬. Smoking 🚬 anything could cause blindness and I’m not joking 🙃.     I used to clean two of Dr. Denton Cooley's family vacation homes in Galveston Texas in the 1980's. I never met him or the family but I cleaned those houses with my Aunt Mary Dabney for a couple years. I always enjoyed working in his houses. I know they have nothing in common but fame and fortune but I also used to clean a vacation home for Willie Nelson some years later. Early to mid 1990's.

    I lost my father to lung cancer. He only had one lung he lost his right lung when he was only 14 years old he fell through the roof of an air raid shelter. After his operation to remove his lung he became gravely ill. It was an ex Army PT instructor who encouraged him back to health and physical fitness he had a beautiful healthy looking body we all loved him very much. He also played football and in 1953 would come home with his pockets full of cigarettes he was a non smoker. Sadly at age 27 he gave into the constant pressure to smoke and drink alcohol. He was only 50 when our beautiful father was diagnosed with lung cancer. I helped my mother nurse him until he passed away at age 51. I am 70 now and I still miss him everyday that love and longing to see him again will be with me till I die. Please if you are tempted to smoke don’t do it. Smoking not only kills the smoker it also breaks the hearts of the people who love them. Your body is a precious gift value it like you would a box of gold.

    A simple and very effective way to treat a leg ulcer is the DermaDomus wound cover. Or the PodoPhylus antimicrobial insole for foot ulcers. It has a controlled release of iodine gas over the ulcer without touching it. It works very well and fast!..

    Just read an article from Nature Medicine 2001. It says “Nicotine Grows New Blood Vessels”. Go figure. Maybe we should be more worried about the poisons from the FDA.

    It will probably take 2 years for this to reverse. To speed this up, exercise often, cardio to up the bloodflow 2 to 3 times a week. Drink plenty of water which also improves bloodflow and eat lots of vegetables and fruits so your cardio and non smoking blood has loads of nutrients in it. Walk a lot too.

    I developed three brain aneurysm's because I have been a long term heavy smoker, two of which I had treated by clipping by pterional approach, craniotomy. I still have one more little ticking time bomb in my brain and still at risk at developing more. Please, don't smoke!

    I think Doctor's should make people who are at a higher risk for blocked arteries aware of having an ultrasound done on them. Since I am a smoker, I asked my pcp to refer me for an ultrasound on my carotid artery. I found that it was severely blocked. I had the surgery, and while doing so they checked my legs and my right leg had two major blockages. While doing my leg they did an ultrasound of my groin area. I found out that my iliac arteries on both sides were pretty blocked also. I just recently had both iliac arteries done and have never felt better. I could not walk a city block before the surgeries as the side of my butt cheeks severely hurt. In my leg, I would have thigh and calf pain. I just had my last iliac artery done a week ago and can walk for the 1st time pain free in over 3yrs. I think anyone who is at risk needs to be checked. Thank god for vascular surgery, I feel like I have gotten my life back.
